Infertility in men is usually caused by the inability to produce quality sperm. In reproductive support treatments such as in vitro fertilization, sperm volume, shape and concentration are evaluated with sperm analysis. However, routine sperm analysis is not sufficient to analyze sperm at a molecular level. DNA damage, also known as sperm DNA fragmentation, prevents the healthy development of the embryo in the uterus and can cause miscarriage. For this reason, sperm DNA damage testing can be used in in vitro fertilization treatment.

What is Sperm DNA Damage?

Myoma treatment  ; Myomas are benign tumors that occur in the uterus and are caused by the proliferation of muscle cells. Myomas, which are one of the most common diseases in women, are most commonly encountered between the ages of 30-40. Myomas, which are thought to occur due to hormonal conditions, have a 1 in 1000 chance of becoming cancerous. Myomas, which usually do not cause any complaints and do not show any symptoms, are detected during routine gynecological examinations.

What is Myoma?

Myomas are usually benign tumors that originate from smooth muscle cells or fibrous tissue cells in the uterus or uterine wall. Myomas, which are rarely encountered in adolescence and young adulthood, usually occur in women between the ages of 30 and 40. Intrauterine myomas, which are seen in 20% of women, are detected by gynecological examination methods such as ultrasonographic imaging. Myomas, which are thought to occur due to the estrogen hormone, also occur due to genetic factors.

Myomas can be found in the uterus, on the uterine wall, or in the abdominal cavity attached to the uterus by a stalk. They can be too small to be seen with the naked eye, or they can be large enough to cause expansion in the uterus. With menopause, myomas usually start to shrink in size due to decreased hormone levels. Myomas that grow after menopause need to be monitored because they carry the risk of becoming cancerous.

What are the symptoms of uterine fibroids?

Myomas, which are generally benign and harmless and do not cause any symptoms or discomfort, are detected during routine gynecological examinations. Myomas that cause complaints in the patient are;

  • Having a painful menstrual period,
  • Long and heavy bleeding menstrual periods,
  • Pain during sexual intercourse,
  • Sudden pain in the abdomen and groin,
  • Feeling of pressure in the abdomen,
  • Frequent urination or urinary incontinence,
  • Constipation,
  • They cause signs and symptoms such as infertility.

Myomas that form on the front wall or top of the uterus can cause pain during sexual intercourse. However, only 1% of myomas cause infertility. If a myoma is seen in the inner layer of the uterus, where the embryo should be placed, the fertilized egg may not be able to attach to the uterus. Due to this condition, patients may develop infertility.

Contrary to popular belief, myomas do not turn into cancer; in other words, myoma is not a sign of cancer. However, in order to distinguish sarcoma cancer, which can occur in the uterus, from myoma, a tissue sample must be subjected to microscopic examination. The difference between sarcoma and myoma may not be apparent in ultrasound imaging. Cancer can be diagnosed when a tumor thought to be a myoma is actually a sarcoma. This can only be done by examining the tumor cells under a microscope. Sarcoma grows rapidly and causes severe bleeding. In particular, single and large myoma masses detected over the age of 40, observed to grow rapidly and cause severe bleeding should be surgically removed in order to eliminate the risk of sarcoma.

How is Myoma Treatment Done in  ?

Myoma treatment is performed according to the complaints caused by the myoma in the patient, the location of the myoma, and its size. In cases of myomas that are detected incidentally and do not cause any complaints, no other treatment is usually applied, except for routine check-ups. In cases of myomas that cause irregular, painful, or very heavy bleeding periods or infertility, surgical intervention is performed. Myoma surgery can be performed laparoscopically, closed or open surgery. After surgical removal of the myoma, there is a decrease in pain and bleeding and an increase in pregnancy rates.

Laparoscopic Myomectomy

It is a closed myomectomy surgery. During the surgery, no large incisions are made, the surgery is performed with 3 or 4 incisions, the largest of which is 1 cm long. A tube-shaped tube with a camera on its end is placed into the patient’s abdomen through an incision at the level of the belly button. This camera allows the surgeon to see the tissues. Myoma tissue is removed with surgical instruments placed inside through other incisions. Performing the surgery with smaller incisions significantly shortens the patient’s recovery period. There is less pain and bleeding risk after the surgery. In addition, the scars after the surgery cover a much smaller area.

Myomectomy with Laparotomy

It is an open myomectomy. It is performed by making a horizontal incision of approximately 10 cm in length in the patient’s abdomen. Since there is a wider and deeper incision area, the risk of developing complications after surgery is higher than with the laparoscopic method. The patient’s recovery and return to normal life after surgery is longer.
