Myoma is the name given to tumors that form in the uterus, mostly benign. Myomas, which form in the smooth muscles in the uterus, occur as a result of the proliferation of muscle cells. Myomas, which are seen in one in every four women, constitute one of the most common gynecological diseases. Myoma formation is usually seen after puberty, when the reproductive age is entered, and the size of the myoma decreases with the onset of menopause. • Growing towards the inner layer of the uterus,
• Found in the intrauterine muscle tissue,
• Growing outside the uterus,
• Pedunculated myomas, which are connected to the uterus by a stalk in the abdominal cavity, and
• Myomas developing in the ovaries
• They are divided into intraligamentary myomas, which are located between the uterine ligaments.

What are the symptoms of myoma?

Myomas usually do not cause any symptoms and are detected accidentally during routine gynecological check-ups. However, in some cases, myomas are seen to cause the following symptoms:

• Excessive menstrual bleeding,
• Severe pain during menstruation,
• Pain during sexual intercourse,
• Sudden cramp-like pain in the abdomen and groin,
• Feeling of pressure in the abdominal cavity,
• Difficulty holding urine or frequent urination,
• Difficulty in defecation or constipation,
• Infertility.

Myoma symptoms may vary depending on the location in the uterus. For example, fibroids located on the anterior wall or the top of the uterus may cause pain during sexual intercourse. Myomas located in the inner layer of the uterus may block the place where the fertilized egg should be implanted, preventing the embryo from implanting. In addition, fibroids that grow due to increased estrogen levels during pregnancy may cause miscarriage.

What is the Treatment for Myoma?

The method to be followed in myoma treatment is planned by evaluating the patient’s history, the patient’s symptoms and the patient’s medical condition. In case of myomas that are detected incidentally and do not cause any complaints in the patient, the monitoring method is used. Regular check-ups determine whether the myoma is growing or not. • Detection of the presence of a rapidly growing myoma during follow-up,
• It is the preferred treatment method in patients planning pregnancy and in the presence of submucous myomas that prevent the embryo from attaching to the uterus.

Before myomectomy, the size and exact location of the myomas should be determined by ultrasound. In the presence of multiple myomas, magnetic resonance imaging, also known as MRI, is used.

How is Myoma Surgery Performed in  ?

Myomectomy surgeries are performed using two different methods: closed and open. Closed surgery is generally preferred due to the smaller incisions required during surgery and shorter recovery time after surgery.

Laparoscopic (Closed) Myomectomy: It is performed by making 3 or 4 incisions, each 1 cm, on the patient. The patient is under general anesthesia during the surgery. A laparoscopic camera is placed into the abdomen with the help of a small incision made in the belly button area. The advanced technology and surgical instruments used are used to remove the myoma from the uterus.

Myomectomy with Laparotomy: Also known as open myomectomy, this method involves making an incision of approximately 10 cm in length in the lower abdomen. The uterus is accessed through this incision and the myoma is removed.
